Cognition, Memory and Energy

As I mentioned last month, I attended the Spring Congress of the American Academy of Anti-aging Medicine entitled: Brain Awakening. One of the sessions featured Dr. Joseph C. Maroon, Heindl Scholar and Professor of Neurosurgery at the University of Pittsburgh Medical Center. Dr. Maroon spoke on Alpha-GPC and Omega-3 for improving cognition, memory and energy.

Alpha GPC (L-alpha-glycerylphosphorylcholine) is found in egg yolk, red meat and organ tissue, but otherwise is scarce in the diet.

Alpha-GPC is used as a precursor for both acetylcholine synthesis and phospholipid synthesis in the brain. Alpha-GPC as a supplement given either orally or by IV consistently influences brain function for 24 hours after administration.

Acetylcholine plays a role in arousal, attention, learning, memory and motivation.

Acetylcholine is made by combining choline (Alpha-GPC provides choline) and Acetyl CoA.

Alzheimer’s Disease is associated with lessened choline uptake and diminished Acetylcholine production contributing to memory loss.

In 2003, Dr. Moreno showed that using the supplement Alpha-GPC resulted in cognitive improvement in mild to moderate Alzheimer’s dementia.

Mean choline intakes for older children, men, women and pregnant women are far below the adequate intake our bodies need. Humans can produce choline in the liver, but the amount that the body naturally makes is not sufficient to meet human needs.

Choline-deficiency impacts atherosclerosis (through regulation of homocysteine) and neurological disorders.

Choline used as a treatment has been shown to limit neurological damage in stroke patients, improve retinal function in glaucoma and to be beneficial in the treatment of  dementia and head trauma.

In summary, Alpha-GPC enhances memory, boosts energy, is a neuro-protectant (supports the development of new brain cells and enhances the body’s ability to repair damaged cell membranes) and stimulates the production of human growth hormone.

The number of brain connections (synapses) formed depend on levels of three key nutrients: uridine, omega-3 (DHA), and choline.

Both EPA and DHA provide anti-oxidant, anti-inflammatory and gene activation to protect neuronal structure and function. DHA increases the production of LR11, a protein that is found at reduced levels in Alzheimer’s patients. LR11 is known to destroy the beta amyloid protein plaques associated with Alzheimer’s Disease.

Omega-3 (both EPA and DHA) have significant structure and function benefits for brain health. DHA contributes to reduce brain inflammation and improve neurotransmitter transmission resulting in clinically improved cognition and memory function.

Alpha-GPC is a unique dietary supplement that rapidly delivers choline to the brain across the blood-brain barrier. Alpha-GPC has been used clinically for its cognitive benefits for Alzheimer’s disease and dementia.

Dr. Maroon has developed a uniquely formulated Alpha-GPC with an Omega-3 that can provide numerous cognitive and brain health benefits when combined versus taking separately. Brain Awakening

In April, I attended the Spring Congress of the American Academy of Anti-aging Medicine entitled: Brain Awakening. One of the sessions featured the American psychiatrist, Dr. Daniel G. Amen speaking on how to supercharge your brain, reverse memory loss and remember what matters most.

Dr. Amen has developed a unique way of imaging the brain to determine brain health. Dr. Amen has considered: what if mental health was really brain health?

Having studied over 135,000 brain scans, Dr. Amen reached the following conclusions:

  1. You are not stuck with the brain you have.
  2. You can make it better – even if you have been bad to it.

To strengthen your brain or get it back, you have to prevent or treat the 11 major risk factors.

Almost all risk factors are preventable or treatable.

Risk factors:

1.  Blood flow –low blood flow is the #1 brain imaging predictor of Alzheimer’s
Hurts –smoking, coffee, heart disease, stress
Helps – diet, ginkgo, exercise, oxygen

2.  Retirement – when you stop learning, your brain starts dying.

3.  Inflammation

4.  Genetic Risk – is not a death sentence, it’s a wake-up call.

5.  Head trauma

6.  Toxins – includes alcohol, cocaine and marijuana.

7.  Mental Health – risk factors – chronic stress, emotional trauma, grief, depression, bipolar disorder, ADD and addictions.

9.  Neurohormone Deficiencies – critical role of:
Testosterone – mood, motivation, sexuality, strength
Thyroid – energy and mental clarity
DHEA – fight aging
Estrogen and progesterone – boost blood flow

Breast Cancer: Prevention vs. Early Detection

As I was preparing for a recent talk on hormones, I came across a rather startling statistic: the incidence of death from breast cancer.  Depending on which statistic you look at, mortality rates have either not changed at all over the last 20 years, or have decreased by up to 44 per cent. Published data from the Canadian Cancer Society in 1989 estimates that the mortality rate for breast cancer was 24 per 100,000 people. According to their latest statistics the projected mortality rate for 2018 is predicted to be 23.2 per 100,000 people.

These statistics are worrying, as breast cancer is the third most commonly diagnosed cancer in Canada, accounting for 13% of all cancers and 25% of all cancers among females. The Canadian Cancer Society indicates that a woman’s lifetime risk of developing breast cancer is 1 in 8. This figure has remained relatively constant for years.

Most of the scientific focus remains on early detection of the disease, as opposed to prevention. It is thought that by the time breast cancer is detected in a woman, it has been present for at least 10 years. It takes this long for the number of cancer cells to grow to the point of being detected. Wouldn’t it be amazing if there were steps you could take now to prevent a cancer that could develop ten or more years later? Looking at some of the treatment recommendations currently being made, there is appreciation for the role estrogen can play.

Did you know that high-dose estrogen (yes, you read that correctly) was the endocrine treatment of choice in postmenopausal women with advanced breast cancer prior to the introduction of tamoxifen in the 1970’s? Tamoxifen became the preferred treatment because it had the same response as high-dose estrogen but had fewer side effects.

In the coming months, I will look at how a woman of any age can test how estrogen is metabolized in their body, and how a simple urine test can help predict your likelihood of developing breast cancer. I will also discuss steps that can be taken to potentially change your risk of developing breast cancer.

Patient Story: Meredith

When Meredith* (name changed at patient’s request) first came to my office after a recommendation from her pharmacist, she hadn’t had a full night’s sleep in over two years. She was on several medications, but none were working.

Long Term HRT Use

I am often asked by patients when they should stop their hormone medications.

Previously, it had been recommended on the traditional side to try to taper or stop hormone medication after being on them for 5 years. However, many women would notice that either while trying to taper their medications or after stopping their medications that they just did not feel well at all. Their bodies were telling them that they needed what they had been using.

In 2017 there were some important developments with regards to hormones and their benefits over time.

The following study followed women over the course of 12 years and concluded that women on hormone replacement therapy had lower all-cause mortality. What does this mean? It means that women taking hormones had a lower risk of dying than women not on hormones and the ones that were on hormones and not dying from heart disease were not at higher risk of dying from something else, for example cancer. This is a good news story. As one of my cardiology colleagues commented: “How amazing it is that taking hormones makes women live longer!”

With regards to how long to take hormones, there is an authoritative group in the USA called the North American Menopause Society (NAMS). This group looks at all the studies with regards to hormones and issues guidelines for physicians. In July 2017, NAMS released the following change to their guidelines:

Patient Story: Jo-Anne Binns

My name is Jo-Anne Binns, I’ll be 55 next week.
